You may apply online or by downloading a pdf application & registration form. The application deadline for CSOP 2012 is April 1, 2012. Late applications may be accepted with a $50 late fee.

To apply using the pdf Application & Registration Form, please complete the form and return it, with payment, to 500 Shaftesbury Blvd, Winnipeg MB R3P 2N2, Canada. Alternatively, you can also scan and e-mail the completed form to csop@cmu.ca. Your application is not complete and your place in the courses is not reserved until full payment has been received.
